Reach level 30 to unlock the Hard difficulty in both single player and multi-player modes.

Reach level 60 to unlock the Super Hard difficulty in multi-player mode.
Complete all quests under the Hard difficulty setting to unlock the Super Hard difficulty in single player mode.
